Fitness Passport: Annual Survey

We are calling on all staff in your learning community to take part in the 2025 Fitness Passport Annual Survey.

By completing the survey, you will go into the draw to win 1 of 5 fitness wearables, valued up to $500!

The survey gives you the opportunity to provide feedback on your program, including how you use your membership, what you’d like to see added to your program and what resources you’d like us to provide to better support you on your fitness journey.

Non-members and former members are also encouraged to contribute; everyone’s feedback is important to developing our program further.

How to have your say:
